Visit your eyecare professional.
They should complete the referral form and fax/send to the center which is closest to your home.
If not contacted directly, contact the center (see below) and make an appointment with the Occupational Therapist (O.T.) who does low vision evaluations.
Go to your scheduled appointment.
The Occupational Therapist will provide appropriate devices made available by LLVCECT. Also, the O.T. will contact us to further refer you for Meta Glasses.
